I just ordered the Medium. Prior to ordering the medium I measured my head and it was exactly 58cm. Exactly. Fattest part of my head = 58 cm. The medium came, the medium did not fit. The problem, my head was a little too long (forehead to occiput). Had it been a little more circular rather that oval, the size would have been grand. However, I literally could not get the helmet on my head all the way.

I did not use the helmet on the road, I think it was beautiful. I'm sad there are no larges in stock. Hope that helps someone else out there.

They made me rate it. So four stars for looking rad, and potentially comfortable. -1 star for not fitting my head.

Please don't read the following comment as a negative criticism. Anyone who drops $250+ on a helmet they have never physically tried on is a complete badass as far as I am concerned.
Generally speaking, I would say you absolutely have to have a plan B if you're going to purchase a helmet online without ever fitting one in person. I have the same issue as you, my skull did not grow to fit the average bike helmet perfectly, so I have to choose them carefully unless I want it pinching against my forehead and loose on the sides; the sizing label is only good for telling me which option to buy online cheaply after trying one on at the LBS. That's right, I said it.
Some bersexy product lines eg Catlike aren't carried by my LBS, so I don't even bother with them.

I agree you need to try a helmet on, especially when switching manufacturers. But remember Backcountry has a very generous return policy, so you can buy, try and return if the item doesn't fit.
